Records Release

If you would like copies of your SHC medical records, complete the following steps: 

1. Please fill out, print and sign

2. Turn form into the Student Health Center
   • Address: Sonoma State University Student Health Center, 1801 E. Cotati Avenue, Rohnert Park, CA, 94928
   • Fax: 707-664-2925
   • Drop Off: Monday - Thursday 8AM - 5PM and Friday 9:30AM to 5PM when campus is open.
3. Please allow 7-10 business days for records release to be completed once received.
