How to Upload COVID Test Results

Log in to your My Health Portal, click on Downloadable Forms/Uploads in the left-hand menu. Then go to COVID Test Results and click Upload. This will become a part of your SHC electronic health record. If you have questions or concerns, please call the SHC at (707) 664-2921.
