Need to upload your vaccine information?
Log into your My Health Portal, on the home page click on 'Enter my COVID-19 vaccine information,' then enter the dates of your immunization and upload your vaccination proof.
Need to request and exemption or waiver?
Log into your My Health Portal, in the left hand menu click on 'Downloadable Forms/Uploads,' download the appropriate form, fill it out and then upload it.
